What do we do?

Provide skilled nursing care for the following:

• Acutely ill & Post operative care

• Terminally ill

• Patients with chronic illness or disability

• Health promotion support and advice for patients in the community

• Dressings clinic jointly with the practice nurses

Who can refer?

• Patients

• Family

• Carers 

• Friends 

• Neighbours

You do not need to see a GP first
