INTERNET BOOKING FOR REGISTERED

You can now book
appointments with most of the doctors at Hanham Surgery using the
internet. This is a pilot trial initially and not all the doctors will be
available for internet booking. Also, you will not be able to
book other services such as the Treatment room nurses or Health Visitor
baby clinics.
HOW TO REGISTER TO USE
THE SERVICE
To make use of this service,
First, you have to REGISTER with the surgery IN PERSON. No-one else can do
this on your behalf including family members. Registered carers are an
exception to this. You will need to bring along TWO documents, one showing
proof of address (e.g Utility bill or Bank statement/letter etc) AND an
item of photo-ID (e.g passport, driving license, official employment ID). This is to comply with the
Data Protection Act.
There can be no exceptions. Once your details are verified by the
receptionists, you will be given a registration document that will provide
you with essential information you then need to use to CREATE an account
on-line.

You will need to enter the
numbers on your registration document to be successful. You will be given
the chance to create a unique password to replace the PIN number on the
registration document. Please try to remember it as you will have to
register again if you lose it.
MAKING AN APPOINTMENT
Once you have completed
this, then you can proceed with making your internet booking by clicking
the following link: MAKE

Only available appointments
will be shown.
If you lose your password or
your access-ID then you will need to re-register. You will have to call in
person to collect a new registration document. NOBODY else can collect it
on your behalf. You will need your address/ID documents again.
